The Case Management Department assists patients and their families with the arrangements and plans for discharge. This includes gathering information regarding the patient's needs such as home health services, durable medical equipment, physical therapy, etc. Case Management can also arrange for discharge to another facility for additional care or skilled nursing care. The Case Management Department partners with many community agencies and has resource lists and contacts for patients and their families.
